Privacy Notice for Chat to Shop
Purpose
Completion of this form is required to ensure that necessary and up-to-date customer information is gathered for the fulfillment of orders made through National Book Store’s Chat to Shop service.
Data Privacy Agreement
By accomplishing this Chat to Shop form, I hereby authorize the National Book Store Group, its affiliates, and/or the accredited third party provider to collect and process the data indicated herein for the purpose of order fulfillment as part of the requirements in the processing of Chat to Shop orders. I understand that my personal information is protected by RA 10173 or the Data Privacy Act of 2012 and that this form will be destroyed after 30 days from the date of accomplishment, following the National Archives of the Philippines protocol.
Personal Data Collected
We collect the following personal information from you when you electronically fill out the Chat to Shop form:
- Email Address
- First Name
- Last Name
- Contact Number
- Home Address
- City / Municipality / Province
Please ensure that all personal information you submit to us is accurate, complete, and up-to-date.
Use
The personal information collected is used for Chat to Shop order fulfillment and/or the accredited third party provider to collect and process the data indicated herein for the purpose of order fulfillment as part of the requirements in the processing of Chat to Shop orders.
Protection Measures
Only authorized National Book Store Group and third-party provider personnel have access to these personal information, the exchange of which will be facilitated through email and/or hard copy. Digital information will be stored in a database while hard copies will be stored in a safe/vault/locked cabinet and both will be stored for as long as you are a Teacher Incentive Program member or 10 years after the last transaction date. Thereafter, physical records shall be disposed of through shredding, while digital files shall be deleted.
Access and Correction
The National Book Store Group’s decision to provide such access to your personal information or consider any request for correction, erasure, and objection to processing of the personal data provided or as it appears in the National Book Store Group’s records is always subject to any exceptions under applicable laws and/or the Data Privacy Act.
